Clinical Summaries and Factual Reports

At TeleThoughts, our psychiatrists may provide brief clinical summaries or factual reports when needed for ongoing care, communication with general practitioners, or administrative purposes.

These reports include information based only on the patient’s clinical consultations and medical history available at the time. TeleThoughts does not provide independent medico-legal assessments, expert witness reports, or legal opinion reports for court or insurance matters.

If a formal medico-legal report is required, we encourage patients to discuss this with their GP, who can provide guidance on referral to an appropriate independent psychiatrist.
